Check the folder structure against INSTALL.md: SKILL.md belongs directly inside the named skill folder. Use the documented pilot project location and a fresh session. Record the exact host version and symptoms; do not overwrite another installed release.
The output contains an invented or missing fact
Stop before using the draft. Compare it with the input and review rubric. Record the affected field and a redacted reproduction. A prototype output needs human review; it has no established accuracy guarantee.
What information is useful for an issue report?
Record the skill ID, version, host/surface, model, step attempted, expected behavior, actual result, and a sanitized example. Do not include client documents, personal data, passwords, API keys, or payment card details.
